Service requests
In order to obtain a licence for provision of services of gambling, gambling service providers shall submit to the Lotteries and Gambling Supervisory Inspection a submission accompanied by:
1) information regarding the merchant's name, registration number and legal address;
2) the development plan for next year of operation, indicating the planned activity types, the planned number of employees, the planned types of games and the planned number of gambling equipment;
3) information regarding the creation of gambling services in the actual address;
4) information regarding the contact person (name, surname and contact information — e-mail, telephone number).Receipt of services
